For chocolate milkshakes that we make at our home, we use chocolate whey, plain coconut water, spinach, banana and/or strawberries, and ice. Very good and healthy! ;) YUM! Sometimes instead of chocolate we use Nutella, or peanut butter. Nutella taste just like Chocolate.
Or you could use vanilla ice cream, chocolate syrup, milk and ice. This would be far too filling for me though.
Wow Nikki that is very ambitious of you but the recipe sounds very nutritious . I am glad to see that you are keeping the health of your residents in mind and that they are willing to drink something like this or do you not tell them what is in it. You're quite the special angel here to assist everyone. It takes a special kind of person to deal with seniors - they are not given the respect or credit they so deserve to have lasted this long on this earth. To have witnessed such history is amazing. So many are forgotten about and I try everyday to share RAOKs (Random Acts of Kindness) with any person I see that could use it - thank you again for the great ideas which I hope to spread around the area to help make others know how special they are.

1/2 gallon chocolate milk
14 oz can sweetened condensed milk
8 oz tub of cool whip
Whisk together the two milks
Fold in cool whip
Using an ice cream machine follow the manufacture's directions to chill.
It takes about 30-40 minutes and then serve immediately.
Perhaps not a traditional chocolate shake but they were delicious and easy enough for all to help.
