I would try to explain why Richard is reluctant to accept assistance. People often react with fear or anger when they think their interests are threatened. How about this:
"Richard has been diagnosed with early stage dementia and he is rather angry about the prospect of going into long term care. His feelings have been acknowledged and empathized with to diffuse strong emotions while negotiation takes place. Respite care is being considered as an introduction to long term care."

It is so helpful, inspiring and they give our residents so much fun and the feeling of achievement once we have completed them.
