Hello, I am new to this group but am loving all the information! I start a new job as an Activity Director at an assisted living home this week and this will be a wonderful resource for me!
Question; I am looking for an Initial Client Assessment Form. Is there one on here that I can just print vs creating my own? I need to do an assessment of each resident on their DOB, what past activities they enjoyed, what current activities they like and would like to see, etc. Does something like that currently exist here?

The last two could be some thing you put in a residents room so that when someone comes to visit or care for them they can look at that and have something to talk about
Most likely you will have to adapt and modify these forms to me the particular needs that you have
